The Passionate Physio is committed to collecting, using and disclosing personal information responsibly and only to the extent necessary for the goods and services we provide. We are committed to ensuring your privacy by complying with all laws and regulations under the applicable privacy laws in Canada. Your personal health information is protected in Ontario by the Personal Health Information Protection Act (PHIPA). PHIPA sets out rules for the collection, use and disclosure of personal health information. This document describes our privacy policies.

Collection of Personal Health Information:

Personal Health information is information about an identifiable individual. This may include, but is not limited to, information relating to personal characteristics (e.g., sex, age, home address, phone number) and health history (e.g., previous or current health conditions/diagnoses, health services, or healthcare practitioners).

During this time, The Passionate Physio collects first name, last name, pronouns, and e-mail address using the optional Newsletter subscription opt-in form and the Contact form at www.thepassionatephysio.ca . She also collects further information for her waitlists & testimonials, including pregnancy/postpartum status and related questions. Further information is collected and appointments are booked through a separate platform, Embodia, which is PHIPA-compliant and its own separate entity, linked through this website.

Your Personal Health Information will be used and disclosed only for the following purposes:

  • To send out newsletter and updates that you can unsubscribe to at any point (detailed below)

  • To contact you, if needed, and with your consent

  • As otherwise permitted or required by law.

  • With verbal and written consent, to share your testimonial, first name and last initial, and picture on this website and social media. Consent can be withdrawn at any time, in writing, by emailing surabhi@thepassionatephysio.ca

We will not sell, trade or transfer your personal information to third parties unless we provide you with notice, except in the circumstances set out below. We reserve the right to release your personal information in order to comply with certain laws or regulations, enforce our site policies, or to protect our own and others’ rights, property or safety.

In the event that the integrity, confidentiality or security of your personal information is compromised, the Chief Privacy Officer is committed to notify you.

Storage Period of Personal Information:

We will store in our password-protected computer systems, in reasonable security conditions, the entirety of the personal information collected for the following duration: 10 years.

Hosting of Personal Information:

Our website is hosted by Squarespace.com and email newsletters are hosted via ConvertKit.

Your Rights:

You can withdraw from the email newsletter subscription at any time. In order to do so, you can submit a request in writing via e-mail to surabhi@thepassionatephysio.ca or unsubscribe directly using the ConvertKit link you received in your email when you first signed up.

Changes to Privacy Policy:

We reserve the right to modify our Privacy Policy in order to guarantee its compliance with the applicable law. You are therefore invited to regularly consult our Privacy Policy to be informed of the latest changes.
